Explanation
BACKGROUND: The Board of Health has been awarded a grant from the Ohio Department of Health to fund the Women, Infants and Children (WIC) Program for the period October 1, 2008 through September 30, 2009. The Ohio Department of Health has provided $3,696.03 for a one month modification and extension of a lease of real property to be used as clinic space for the WIC Program for the period October 1, 2008 through October 31, 2008. The WIC program needs to continue to occupy space at 1120 Morse Road for an additional month while the new clinic locations are configured.

To authorize and direct the Director of Finance and Management Department to modify and extend a lease contract with Four D Holdings for the lease of clinic space for the WIC program, for the period October 1, 2008 through October 31, 2008; to authorize a total expenditure of $3,696.03 from the Health Department Grants Fund; and to declare an emergency. WHEREAS, the City desires, for the benefit of the Department of Health, to modify and extend a lease contract by one month, with Four D Holdings, for the rental of real property containing 5,241 square feet of space at 1120 Morse Road, Columbus, Ohio for the continuation of Women, Infants and Children program services; and,
WHEREAS, an emergency exists in the usual daily operation of the Health Department in that it is immediately necessary to modify and extend a lease for the immediate preservation of the public health, peace, property, safety and welfare; Now, therefore,
