Title: To authorize and direct the Board of Health to modify and increase a contract with Stericycle, Inc. for the provision of infectious waste disposal services for the period of March 1, 2014 through February 28, 2015; to authorize the expenditure of $3,000.00 from the Health Department Grants Fund, and to declare an emergency. Explanation

BACKGROUND: The City of Columbus currently contracts with Stericycle, Inc. for the provision of infectious waste disposal services for its various clinics. Disposal services include providing the necessary boxes, bags, and ties, pick up and transport services, destruction and disposal of the waste, and manifest reporting with distruction certification. The original purchase order (ED050098) was established February 27, 2014 using Health special revenue funds. The contract compliance no. for Stericycle is 363640402. This ordinance authorizes a modification and $3,000.00 increase to the current contract for the period through February 28, 2015. This additional support is needed for the ongoing infectious waste disposal from Columbus Public Health Women, Infant, and childres (WIC) clinics.

Total contract amount including this modification: $15,500.00

Emergency action is requested in order to avoid any delays in providing program services.

FISCAL IMPACT: This contract is funded by both the Health Special Revenue Fund and the Health Department Grants Fund.

WHEREAS, Columbus Public Health has a need for infectious waste disposal services; and,

WHEREAS, in order to ensure continued service provisions, it is necessary to modify and increase a contract with Stericycle, Inc. for the provision of additional infectious waste disposal services; and,

WHEREAS, the contract period is March 1, 2014 through February 28, 2015; and,
