Explanation
BACKGROUND: This legislation authorizes the Director of the Department of Technology to modify and extend a contract established with purchase order EL000490, for license and support services, for Envision for Windows software product, on behalf of the Columbus Health Department, provided by Decade Software Company LLC. Envisions for Windows is a critical application that is owned and copyright by the Decade Software Company. This service provides the Health Department the ability to operate the Environmental Health Unit, which provides an extensive array of services such as; tracking and maintaining inspection and historical data of restaurants and other businesses, applications, accounts receivable/ payable and billings, tracking of data regarding sanitation standards, factor control for mosquitoes, environmental reviews, statistics and analysis regarding the Environmental Health Unit. Services provided under this agreement include telephone support, software maintenance, update training, upgrades and enhancements. Without this Software License and Support Services the Health Department would lose the ability to operate the Environmental Health Unit, which could result in the failure of meeting requirements mandated by the State Of Ohio.

This contract modification and extension is a license and support services contract in the amount of $43,260.00, which will continue to enforce all terms and conditions provided under purchase orderEL000490 from July 1, 2003 until June 30, 2004.

Emergency: There is an immediate need to modify a contract established by purchase order EL000490 to maintain on-going support, software license and to allow for uninterrupted daily operational services provided by the Environmental Health Unit.

Fiscal Impact: Fiscal year 2002, $43,260.00 was expended toward services provided by Decade Software for the Health Department. Funding was budgeted and is available in the 2003 Department of Technology, General Fu...
