Title: To authorize the Director of the Department of Development to execute a grant agreement with Physicians CareConnection in an amount up to $30,000.00; to authorize the provision of services for a 16 month period from August 1, 2023, to December 31, 2024; to authorize the advancement of funds on a pre-determined schedule during the term of the agreement; and to authorize the transfer and expenditure of up to $30,000.00 from the General Fund. Explanation

BACKGROUND: This legislation authorizes the Director of the Department of Development to execute a grant agreement with Physicians CareConnection for a 16 month period from August 1, 2023, to December 31, 2024, in an amount up to $30,000.00, using general funds from the Elevate 2.0! Program.

Physician’s CareConnection (PCC) is a volunteer-based, charitable organization that helps coordinate care for vulnerable adults in Central Ohio. In 2021, PCC began a pilot program, called the Westside Assistance Collaborative, with the purpose of improving the social determinates of health for Westside families in Columbus/Franklin County. This pilot focused on using evidence-informed practices to enhance food security and housing stability. The collaborating service partners in this initiative are Community Mediation Services of Central Ohio, The Legal Aid Society of Columbus, Mid-Ohio Food Collective, and Physicians CareConnection.

Applying the learning from the pilot phase, the partners wish to continue to evolve this program to reach more Westside families. The primary population focus are Westside families residing in/or engaged in zip codes 43228 and 43204. Families with children enrolled in school, including pre-K, who must leave current housing or who may have to leave their current housing in the foreseeable future.

The City of Columbus Department of Development seeks to fund the Program Coordinator role of this initiative with a one-time grant award as a social innovation initiative of Elevate! The tasks of the Program Coordinator are to facilitate program improvements sessions with partners, compile program data, package progress reports, design and facilitate monthly improvement sessions with project partners, and prepare quarterly progress reports. For the purpose of this grant agreement, PCC will act as the lead agency.
