Explanation
Background
The Central Ohio Area Agency on Aging, in partnership with Lifecare Alliance, was awarded federal grant funding for a second year under Title IV of the Older American's Act to provide chronic disease self-management workshops to participants in Central Ohio.
This legislation will modify and extend the contract with Lifecare Alliance to provide said workshops in accordance with the proposal submitted to and approved by the Ohio Department of Aging.
These workshops will be given in community settings such as senior centers, churches, libraries and hospitals to assist people in managing different chronic health problems such as arthritis, diabetes, high blood pressure and cholesterol level.

Emergency action is requested in order for the workshops to be continued beyond July 31, 2007 without interruption, in accordance with the conditions of the grant.
Fiscal Impact
$35,940.00 is required and budgeted from the Recreation and Parks Grant Fund to meet the financial obligation of this contract modification.

WHEREAS, an emergency exists in the usualy daily operation of the Central Ohio Area Agency on Aging of the Recreation and Parks Department in that it is immediately necessary to modify and extend said contract in order for the services so that there is no interruption of services; now, therefore
