Title: To authorize the Finance and Management Director to enter into contracts for the option to purchase Miscellaneous Medical Supplies on an as needed basis with Bound Tree Medical LLC, Midwest Medical Supply, and Moore Medical, LLC; to authorize the expenditure of Three dollars to establish the contracts from the Mail, Print Services and UTC Fund; and to declare an emergency. Explanation

BACKGROUND: This ordinance authorizes the Finance and Management Director to enter into a contract for the option to purchase Miscellaneous Medical Supplies on an as needed basis for use by the Department of Public Safety, Division of Fire EMS personnel. These items are used to re-supply the emergency squad units throughout the city. This bid was coordinated with the City of Westerville so that they may issue their own contracts from the responses to this bid. Items bid were divided into six Groups of like products. The term of the proposed option contracts will be through June 30, 2015 with the option to extend these contracts for One (1) additional year. The Purchasing Office opened formal bids on May 3, 2012.

The Purchasing Office advertised and solicited competitive bids in accordance with Section 329.06 (Solicitation No. SA004343). Forty one (41) Bids were solicited (F1:1). Five (5) Bids were received.

The Purchasing Office is recommending award to the lowest, responsive, responsible and best bidders by group, responsive bidders having bid all products in a group:

Bound Tree Medical, LLC CC#311739487 (expires 5-10-2013) Groups 1, 2, 4 and 5
Midwest Medical Supply., LLC CC#431741196 (expires 4-25-2014) Group 3
Moore Medical, LLC CC#202046702 (expires 11-26-2012) Group 6
Total Estimated Annual Expenditure groups 1, 2, 4 and 5 $550,000.00
Total Estimated Annual Expenditure group 3 $300,000.00
Total Estimated Annual Expenditure group 6 $150,000.00

These companies are not debarred according to the Federal Excluded Parties Listing System of the Federal Government or prohibited from being awarded a contract according to the Auditor of State Unresolved Findings for Recovery Certified Search.

This ordinance is being submitted as an emergency because the current contract is set to expire on June 30, 2012 and without emergency action the ability of the City to restock its' emergency medical vehicles will be reduced. The purchasing offic...
