Explanation

 

BACKGROUND:  This ordinance is needed to modify, increase and extend contract ED053773 in the amount of $3,100.00 for the total contract amount not to exceed $11,335.00 for a time period ending December 31, 2016 with Confluence Unlimited.  This modification is needed to provide continued design and facilitation of four additional stakeholder meetings for the Chronic Disease Prevention Advisory Board and subcommittees. The Chronic Disease Prevention Advisory Board is a collaboration of decision makers from a network of organizations representing community-based organizations, healthcare, schools and worksites who work together to identify ways to reduce chronic disease throughout Columbus. As a result of the facilitated Chronic Disease Prevention Advisory Board meetings, participants will build relationships and collaboration among one another to support future work of chronic disease prevention in Columbus.

 

Funds are made available for this contract increase by the 2016 Creating Healthy Communities Grant, through the Ohio Department of Health, which focuses on the utilization of population-based strategies to address healthy eating, active living and smoke free living, with the goal of reducing chronic diseases.

 

FISCAL IMPACT:  This contract increase is entirely funded by the grant award from the 2016 Creating Healthy Communities Grant through the Ohio Department of Health. The original contract period of September 1, 2015 through April 30, 2016 will be extended an additional eight months through December 31, 2016.

WHEREAS, it is necessary to modify contract ED053773 with Confluence Unlimited by increasing the contract by $3,100.00 and extending these services through December 31, 2016; Now, therefore,

 

 

BE IT ORDAINED BY THE COUNCIL OF THE CITY OF COLUMBUS:

 

 

SECTION 1. That the Board of Health is hereby authorized to modify, increase and extend contract ED053773 with Confluence Unlimited by adding an additional $3,100.00 to the contract, for a total amount not to exceed $11,335.00 for a time period of May 1, 2015 through December 31, 2016.

 

SECTION 2.  That the expenditure of $3,100.00 is hereby authorized from the Health Department Grants Fund, Fund No. 2251, Division No. 5001, in Object Class 03, Services, per the accounting codes in the attachment to this ordinance as follows:

SECTION 3.  That the funds necessary to carry out the purpose of this ordinance are hereby deemed appropriated, and the City Auditor shall establish such accounting codes as necessary.

 

SECTION 4That the City Auditor is authorized to make any accounting changes necessary to ensure that this contract is properly accounted for and recorded accurately on the City’s financial reports.

 

SECTION 5.  That this ordinance shall take effect and be in force from and after the earliest period allowed by law.
